Your first visit

On your first visit to Brisbane Cardiology, we will collect patient information including name, contact details, Medicare details, referring physician and next of kin information.

Our cardiologists will acquire a comprehensive account of your current symptoms, previous conditions and all relevant medical history.

Due to the information that needs to be obtained, this appointment may take slightly longer and as such please allow extra time.

To help us complete our records, please bring with you:

• GP referral letter,

• Medicare Card,

• Concession card (if applicable),

• Private Health Insurance Card (if applicable),

• Current medication list

• Recent test results including pathology and X-ray.
